The move from a hospital or rehabilitation center back to the home is a delicate time for older adults and patients who are chronically ill. In particular, the 30 days following a hospital or care center discharge are the most tenuous, as many patients do not receive proper care, are confused about their medications, are uneasy about being alone, and are frequently readmitted to the hospital during this time.

Our caregivers can create a smooth transition for our clients, filling those care gaps and helping to prevent unnecessary re-hospitalizations or going to the local emergency room in a panic.
Our Hospital-to-Home program includes the following:
- An RN to set up and reconcile medications
- A Certified Nursing Assistant (CNA) for 8 hours to attend to your loved one’s personal care needs, including:
- Ambulation assistance
- Transferring assistance
- Bathing and toileting assistance
- Meal preparation and hydration support
- Laundry and linen changes
- Medication reminders
- Light housekeeping and refrigerator clean out
- Safety and comfort assurance
